Cannot open any Control Panel icon
The cache is saved on shutdown and reloaded on Startup so can take months to refesh itself. To refresh the icons in the system change the size of icons in Start - Control Panel - Display - Appearance - Advanced, click apply then change the size back. This is what tweakui does to repair icons.

If I go into the Tweak UI control panel and choose "Rebuild Icons" (on the "Repair" tab) the problem is resolved... until the next time I log in. I've also tried resetting everything on the "Explorer" tab to 'factory defaults'. Anybody else ever seen this? Is there an easy fix??
